Authorization to Charge

By checking the box below and clicking “Pay Now”, I authorize 9jaTelemedicine Limited to charge my selected payment method the following:

• Consultation Fee: ₦3,500.00
• Applicable Paystack payment processing/service charges, where applicable
• Any taxes or statutory charges required by law

I understand that payment will be processed securely through Paystack, an independent third-party payment service provider.

I acknowledge and agree that:

1. My payment information is processed by Paystack and not stored by 9jaTelemedicine Limited except as necessary to maintain payment records.
2. The consultation fee is payable before my medical consultation is submitted to a physician.
3. Payment does not guarantee that:
   • A diagnosis will be made;
   • Medication will be prescribed;
   • A laboratory test will be ordered;
   • A referral will be issued; or
   • My preferred treatment will be recommended.
4. My consultation will only be reviewed after successful payment.
5. I have reviewed and agree to the Terms and Conditions, Privacy Policy, Medical Disclaimer, Informed Consent for Asynchronous Telemedicine, Refund Policy, and Prescription & Medication Policy.
